ONLINE CONSENT FOR HEALTH SERVICES & CLINIC CARD
Attention SCPS Families!
I am pleased to inform you that our Parental Consent for Name Deviation, Student Code of Conduct Acknowledgement, as well as an online application acknowledgement form will go live on Monday, July 28th for parents to complete in the Skyward Family Access Parent Portal. This new feature allows parents to complete these SCPS Documents in real-time, providing a convenient and efficient way for them to manage items throughout the school year.
Please remember to complete your Parent/Guardian Consent for School Health Services form and Student Clinic Card, which can be found in Skyward Family Access. The school health services consent form and student clinic card is automatically uploaded into each parent/guardian Skyward Family Access account and will display at the very top of the home page. To complete the form please follow the steps in attached documents.
